Kitchen sanitation failure with drain flies in food prep areas

Summary

The facility failed to maintain an effective pest control program by failing to exclude and prevent flying insects in the kitchen and dishwashing areas, resulting in direct cross-contamination of food preparation surfaces. On 5/3/2026, surveyors observed flooring throughout the kitchen and dishwashing room heavily soiled with food debris, refuse, dishes, utensils, and containers. In the main kitchen, the floor beneath the three-basin sink was heavily soiled with decomposing and rotting food debris that obscured the floor tiles at the floor/wall junction. In the dishwashing room, the floor and food grinder were heavily soiled with food debris, decomposing food covered the food grinder and the legs and underside of the drainboards, and the wall surfaces beneath the drainboards were black with deposits. Gray stagnant water was present beneath the dishwasher drainboards, and a pile of black rotting food debris was resting against the wall behind a drainboard table leg. Flying insects were observed in the same areas over multiple dates. Four or more drain flies were seen beneath the drainboards, flying between a sewer pipe in a floor drain and the dishwasher drainboard surfaces, and ten or more dead drain flies were floating in puddles of decomposed food saturated with water leaking from the food grinder. On later observations, five or more flies were resting on and flying around the dishwasher drainboard and landing on the drainboard surfaces, and four or more flies were flying between floor drain sewer pipe openings and food preparation table surfaces adjacent to the three-basin sink. The facility's licensed pest control contractor stated the flies in the kitchen areas were due to unresolved sanitation concerns that had persisted for years, and a cook reported that the food prepared in dietary service is available for all residents in the facility.
