Failure to Use PPE and Perform Hand Hygiene During COVID-19 Isolation and Colostomy Care

Summary

The facility failed to wear appropriate PPE and perform hand hygiene during care and isolation precautions for residents with COVID-19 and for a resident with a colostomy on Enhanced Barrier Precautions. R59 stated he tested positive for COVID the prior week and reported that staff had not worn gowns when entering his room to provide care. No container or red biohazard bag was observed in R59’s room or bathroom for disposal of dirty PPE, and R59 stated staff were placing dirty gloves in his trash can. The door sign for R59 indicated COVID-19 high risk of infection beyond the point of entry, but there was no sign identifying contact/droplet isolation or the PPE required for COVID. R1 and R9 also had door signs stating COVID-19 high risk of infection beyond this point and to wear a face covering, but there was nothing indicating contact/droplet precautions or what PPE was required to enter either room. R1 stated she tested positive for COVID after admission and that staff sometimes wore only a mask when entering her room. R9’s room did not have a trash receptacle in or immediately outside the room to dispose of PPE or trash. The line listing documented R1 tested positive on 12/4/25 and R9 tested positive on 12/6/25. During observation of colostomy care for R23, V9, RN, put on clean gloves without washing hands, asked for hand sanitizer, and continued the procedure while changing gloves multiple times without performing hand hygiene between glove changes or after removing gloves. V9 cleansed around the stoma, changed the colostomy bag, and cut adhesive tape while repeatedly donning new gloves without hand hygiene. R23’s record documented chronic kidney disease stage 3, colostomy status, hypertension, chronic diastolic heart failure, and dependence on staff for toileting hygiene and physical hygiene. The DON stated staff are required to wash hands every time they make contact with a resident, before assisting with food, and after touching high-contact areas, and that hand hygiene is expected before putting on gloves, when changing gloves, and after taking off gloves.
