Summary

Ensure food and drink is palatable, attractive, and at a safe and appetizing temperature was not met when breakfast food temperatures were not documented and hot food was served without being checked. On 12/10/2025 at 7:40 AM, the Food Log for breakfast was blank and did not document any food temperatures. At 8:08 AM, the Dietary Manager removed an industrial-size pan of hash browns from the oven, did not take the temperature, placed them in the steam table, and served residents without taking any temperatures. Later that morning, a tray taken after the last resident had been served showed eggs and hash browns that were not palatable and were cold with a rubbery texture; the eggs measured 90.4 F and the hash browns measured 101.0 F with a calibrated metal thermometer. At 9:13 AM, the Dietary Manager stated no temperatures had been recorded in the Food Logbook that morning for breakfast. Resident interviews documented repeated concerns that meals were cold. A cognitively intact resident stated the food was not hot when it was supposed to be hot and that breakfast had been cold the day before. Another resident stated breakfast was cold and that meals were cold a lot in the facility, while others reported food was not always hot, eggs were cold and could not be eaten, and food was usually cold when it came out to residents. Resident and family council minutes also documented complaints about cold food, including cold coffee and food served cold, especially soups. The Dietician stated that all food on the steam table should be held at a minimum of 135 degrees Fahrenheit and that food temperatures below 135 degrees put residents at risk for food borne illness.
