Summary

The facility failed to safely warm up food in the microwave for one resident, resulting in a burn injury when hot noodles spilled onto her lap. The resident had diagnoses including essential hypertension, GERD, kyphosis, cerebral palsy, and a prior burn of the right thigh. Her MDS BIMS score was 15, indicating intact cognitive response. The incident involved the resident receiving hot noodles from staff, then traveling in a motorized wheelchair with a tray table and later entering an elevator where she had a muscle spasm and the noodles spilled onto her right thigh. Documentation states the resident sustained a burn to the right upper thigh where her pants were bunched against the skin. Progress notes describe that wound care assessed her after the spill, pain medication and first aid were provided, and the NP later evaluated the injury as a superficial thermal burn to the right thigh. However, the hospital record later documented the injury as a scald burn with a 1% TBSA burn to the right thigh, and the resident was diagnosed in the ER with a second-degree burn. The wound assessment described the burn as facility acquired, thermal heat, partial thickness, measuring 15.0 cm x 17.0 cm x 0.10 cm. Interviews showed staff were not aware the resident had muscle spasms when the noodles were warmed and transported. The CNA stated she warmed the noodles and placed them on the resident’s wheelchair tray table, then left to get a fork. The RN stated the resident had asked staff to warm the noodles and that the incident occurred on the elevator when the resident had a muscle spasm and the noodles spilled onto her leg. The DON stated she was on the elevator with the resident when the spill occurred and later acknowledged the incident should have been reported that day. The alternate physician stated that based on the tissue description and blistering, the burn went through more than one layer of skin and the resident should have been sent to the hospital.
