Late Medication Administration, Lack of Direct Observation, and Delayed Documentation
Summary
The facility failed to ensure medications were administered within one hour of the scheduled time, failed to ensure the nurse observed the resident while taking the medications, and failed to document medication administration after giving medications. These failures were identified for 6 residents reviewed for medication administration out of a total sample of 13 residents. One resident had diagnoses including spinal stenosis, hemiplegia and hemiparesis, and osteoarthritis, and a BIMS score of 12 indicating moderately impaired mental status. On observation, the LPN prepared the resident’s medications and brought them into the room, but after the resident took one tablet, the nurse left the room to obtain a blood sugar check and the resident continued taking the remaining medications without the nurse present. When questioned, the nurse stated the resident wanted to take a rest, but the resident stated she did not say that. The nurse also stated he had already passed medications for other residents but had not documented them yet and said he was supposed to document right after giving the medications. For the other residents reviewed, the medication administration audit showed late administration times beyond one hour after the scheduled time. One resident with diabetes, osteoarthritis, and schizophrenia had a BIMS score of 3 and had 8 a.m., 9 a.m., and 10 a.m. medications documented between 4:25 p.m. and 5:29 p.m. Another resident with diabetes, hypertension, and adult failure to thrive had a BIMS score of 3 and had a 9 a.m. medication documented at 5:08 p.m. A resident with hemiplegia, diabetes, and dysphagia had a BIMS score of 5 and had 8 a.m. and 9 a.m. medications documented between 5:14 p.m. and 5:49 p.m. A resident with Parkinson’s disease, dementia, and diabetes had a BIMS score of 7 and had 8 a.m. and 9 a.m. medications documented between 5:11 p.m. and 6:00 p.m. A resident with hypertensive heart disease, COPD, and schizophrenia had a BIMS score of 10 and had 8 a.m., 9 a.m., and 10 a.m. medications documented between 5:10 p.m. and 6:10 p.m. The DON stated the expectation was to administer medications one hour before and one hour after the scheduled time, observe the resident swallow and tolerate the medication, and document immediately after administration.
