The facility failed to thoroughly investigate falls and document a root cause for two residents reviewed for falls. One resident experienced a witnessed fall while staff were assisting with activities of daily living in the resident’s room. The resident’s Risk Management Form documented that the resident stood from the bed, began walking with a walker, and fell, but it did not include a witness statement from the CNA identified as assisting at the time of the fall and did not document a possible cause or root cause analysis.
