Summary

Provide appropriate pressure ulcer care and prevent new ulcers from developing was not ensured for a resident with paralysis of the upper limb following cerebral infarction, right hip/buttock stage 3 pressure ulcer, right and left knee stiffness, and muscle weakness. The resident’s assessment showed partial to moderate staff assistance was needed for bed rolling, total staff assistance was needed for putting on footwear, and no cognitive impairment was identified. The resident was also assessed as at risk for pressure ulcer development. The resident had a chronic right lateral hip wound documented as stage 4, measuring 1.5 x 0.4 x 1.0 cm, with the objective to prevent deterioration and recommendations to limit sitting to 60 minutes, offload wounds, and turn side-to-side in bed and reposition per facility protocol. During observations, the resident was seated in a wheelchair with both feet resting directly on the floor while heel protectors were on the bottom shelf of the nightstand. The low air-loss mattress on the bed was repeatedly observed set to the firmest level, beyond the 400-level mark, and the resident stated the mattress was hard, uncomfortable, and made it difficult to move and roll in bed. The resident also stated staff did not float his heels or reposition him in bed, and that the boots come off when he is in bed and staff do not attempt to put them back on. The DON stated the air mattress settings should be based on resident weight and monitored every shift, and that staff should attempt to get the resident to wear heel protectors or float his heels in bed. The resident’s TAR documented heel boots in place on multiple days and air mattress checks every shift, but the survey observations showed the boots were not in use and the mattress remained set beyond the indicated level.
