Failure to Monitor Antibiotic Use and Organism Documentation
Summary
The facility failed to follow standards of practice for antibiotic use for 2 residents reviewed for antibiotic stewardship. For one resident with severe cognitive impairment, urinary incontinence, and a history of urinary issues, the infection preventionist documented a urinary tract infection and ciprofloxacin treatment, but did not have the microbiology report at the time of review. The resident’s progress note showed return from the emergency room with an indwelling catheter and a plan to start ciprofloxacin, and the medication administration record showed ciprofloxacin given for UTI. When the microbiology report was later obtained, it showed Methicillin-Resistant Staphylococcus Aureus with ciprofloxacin resistance. For the second resident, who was cognitively intact but dependent for toileting hygiene and had an indwelling catheter, the record showed a urine culture from the hospital with Escherichia coli probable extended spectrum beta lactamase producer. The facility’s infection surveillance report did not document this resident’s infection, signs or symptoms, status of infection, organism, or antibiotic from the urine culture results. The infection preventionist stated the resident had been sent to the emergency room for confusion and admitted with multiple diagnoses including UTI, but she did not follow up on the organism and stated she does not typically follow up with infections when they are treated in the hospital, even if symptoms started in the facility. The facility’s infection control spreadsheet also showed multiple UTIs over a 6-month period, including 27 UTIs with antibiotics listed and 7 UTIs missing organism documentation to verify whether residents were on appropriate antibiotics. The facility’s antibiotic stewardship guideline stated that the purpose of antimicrobial stewardship is to promote appropriate use of antimicrobials by selecting the appropriate agent, dose, duration, and route, and that the facility will monitor antibiotic use and outcomes from antibiotic use.
