Summary

The facility failed to ensure newly identified pressure wounds were assessed and failed to follow physician orders for treatment of pressure wounds for two residents. One resident had multiple diagnoses including severe protein-calorie malnutrition, Parkinson’s disease, muscle weakness, and existing pressure ulcers and open wounds. On 3/19/2026, nursing documentation described new skin concerns including non-blanchable redness to the left foot and right hip and a blister to the right knee, and the nurse practitioner was notified. The provider note documented erythema to the right hip, erythema and blistering to the right knee, and erythema to bony prominences of the left foot, with concern for newly identified pressure wounds. Staff interviews showed the new skin areas were reported to the wound nurse, but no treatment orders or wound records were in place for those newly identified wounds. The LPN stated the wounds should have been treated as soon as they were identified and measured weekly, and confirmed there was no treatment or record after reporting the skin concerns. The wound nurse stated each wound should have separate treatment orders and should be measured and recorded upon initial identification, but also stated there were no treatment orders for the new reddened areas and blister because they only monitored unopen pressure wounds. The nurse practitioner stated the new areas were newly identified pressure wounds and expected separate wound treatments to be in place for continuity of care. A second resident was admitted with diabetes, Parkinson’s disease, mild protein-calorie malnutrition, an unstageable vascular wound to the left outer foot, and a surgical wound to the left second toe. The admission referral packet and a physician wound-care script dated 2/14/2026 directed cleansing the left second toe and left lateral foot daily and applying a foam pad until assessed by the wound nurse, but no corresponding wound care orders were entered into the electronic record from 2/14/2026 through 2/18/2026. The wound nurse acknowledged that no wound treatments were in place during the first five days after admission, and the facility’s policy required wound treatments per physician orders and full body skin assessments on admission, weekly, and after any newly identified pressure injury, with wound measurements and characteristics documented upon discovery and at least weekly thereafter.
