Summary

The facility failed to maintain a functioning call light system in the bathroom and bathing area for 2 of 5 residents reviewed for environment. R15’s record showed an admission date of 3/3/25 with diagnoses including pneumonia, sepsis, and anemia, and an MDS dated 12/25/25 documented a BIMS score of 11, indicating moderate cognitive impairment. During observation on 2/17/26, R15 and R54’s room had a call light box on the wall with capped off wires and no cord call lights present. R15 was resting in bed with an analog call bell on the bedside table, but when asked, he said he did not have a call light and did not really use the bell. Staff later tested the bell and responded to the room. The Administrator said R15 could have a pendent call light if he wanted but wanted to keep the analog bell. R54’s record showed an admission date of 12/18/25 with diagnoses including fracture of the neck of the right femur, presence of a right artificial hip joint, Alzheimer’s disease, seizures, and need for assistance with personal care. His MDS dated 12/24/25 documented a BIMS score of 5, indicating severe cognitive impairment. During interviews, R54 said he did not have a call light and stated staff would just come in and check on him. The Administrator later said R54 had a pendent call light that hung from a lanyard and rang to a box at the nurse’s station, but when R54 was interviewed again, he said he did not have a call light, thought the pendent might be on his bedside table, and it could not be found. The Maintenance Director said the call light system for R15 and R54 could not be fixed because it was too old and parts were unavailable, and the Administrator said the facility did not have a call light policy.
