Summary

The facility failed to ensure therapeutic diet orders were provided as ordered for 4 of 4 residents reviewed for diet order accuracy. R8 had diagnoses including dementia, type 2 diabetes, and dysphagia, with an order for an easy-to-chew texture and regular/thin liquids. During the lunch meal service, R8 was observed receiving only a small amount of protein, and the Assistant Dietary Manager stated that was all the mechanically altered meat available and that the resident was not a big eater. The Dietary Manager later stated that meatloaf was available and could have been ground up to make more mechanically soft meat, and that staff know to ask for more to be made if they run out of mechanically altered foods. R22, R23, and R26 each had orders for regular diets with double protein at meals, but during lunch meal service they did not receive the double protein portions on their trays. R23 stated she only received one serving of meatloaf and one corndog, and R22 stated he only received one corndog and wanted another. The Dietary Manager stated staff serving and delivering trays should check tickets before delivery, while the Cook and Assistant Dietary Manager stated double protein meant residents should receive double portions of the protein item. R26’s tray on two separate observations contained only regular meal items, and the resident stated she never received a meal tray with double protein portions or a peanut butter sandwich. The Registered Dietitian’s note documented continued regular diet with double protein portions, and the facility’s therapeutic diet addendum stated residents with physician orders for double protein would receive an extra ounce of protein added to the meal.
