The same controlled drug form also documented 6 additional doses given after 5/21/2026 when there was no active physician order, including doses on 5/22/2026, 5/26/2026, 5/28/2026, 5/31/2026, and 6/2/2026. The MAR for 5/1/2026 through 5/31/2026 did not record any administered doses of lorazepam, and there was no documentation in the progress notes of signs or symptoms, administration of lorazepam, or whether the medication was effective. Nursing staff interviews indicated that PRN medications should be checked against an active order, documented on the MAR and controlled drug form, and followed for effectiveness.

The DON reviewed the MAR and stated it was not accurate because staff had different ideas of what restlessness was, and stated the resident should have the correct manifestation of anxiety reflected in the physician order and MAR. The facility policy stated psychotropic medications are to be used only when a practitioner determines the medication is appropriate to treat the resident’s specific, diagnosed, and documented condition.
