Summary

Food service sanitation and infection control practices were not maintained in the kitchen. During the kitchen tour on 6/14/2026 at 9:30 AM, V8 (Dietary Dish Washer) and V9 (Cook) were observed without hairnets while working in the kitchen. The facility’s Personal Hygiene policy states that hairnets, bonnets, or other hair coverings must be worn at all times in the kitchen and must completely cover the hair. The kitchen refrigerator contained multiple opened food items that were not labeled with the date opened, including three small saran wrapped packages of yellow square cheese, one bag of mozzarella cheese, two packs of bologna, a large square of butter, a container of liquid egg thawing, and a condiment squeeze container with a purple substance. The facility’s Labeling and Dating Foods policy requires opened foods and refrigerated items to be labeled with the date opened. At 9:45 AM, V8 tested the high-temp dish washer using chlorine test paper after the wash cycle, and the strip read 10 ppm. The facility’s Dishwashing Procedure policy states the chlorine sanitizer level should be 50 ppm. V7 (Dietary Manager) confirmed that all items in the refrigerator should have a label and open date and that the dish washer chlorine test strip should be at 50 ppm.
