Summary

The facility failed to ensure fall-prevention interventions were implemented for three residents reviewed for falls. One resident was admitted with kidney failure, heart failure, atrial fibrillation, diabetes, and anxiety disorder, had a BIMS score of 15, and was assessed as high risk for falls. His care plan included interventions such as not leaving him unattended in the bathroom, re-educating him to ask for help, and observing for weakness and tiredness, but his fall reports documented falls on multiple dates, including two later falls for which no new fall intervention was documented in the care plan. However, fall reports documented repeated falls in which he was found on the floor near the bathroom or unable to describe how he fell. During observation, his walker was approximately three feet from his recliner and his pressure alarm was on the bedside table on the opposite side of the bed rather than under him. Staff gave differing descriptions of his fall interventions, and the DON stated she would expect new and appropriate interventions to be initiated after a fall and implemented. A third resident was admitted with malignant neoplasm of bone, heart disease, and diabetes, had a BIMS score of 15, and was assessed as not at risk for falls. Her current care plan did not document a fall focus area or interventions. An alleged fall report documented that she was found sitting on the floor after reporting that her walker tipped over and she fell against the door frame and slid to the floor, but no fall intervention was documented in that report. The DON stated the resident had one or two falls since admission and reviewed the current care plan, confirming there was no focus area or interventions in place to prevent falls.
