Summary

The facility failed to implement a comprehensive care plan with interventions for one resident and failed to prevent abuse for another resident. The deficiency involved a resident with diagnoses including COPD, PTSD, panic disorder, anxiety disorder, bipolar disorder, and sleep disorder, who was physically assaulted by her roommate with a cane. The resident reported that the roommate came up behind her while she was arranging her belongings, placed a cane to her throat, pulled back on it, and pulled her hair, leaving a bruise to her throat and worsening her PTSD and anxiety, with nightmares, poor sleep, and avoidance of meals and activities afterward. A CNA witnessed the incident and observed the roommate holding the cane to the resident’s neck and pulling it back as if choking her, then grabbing the resident’s hair when staff intervened. The roommate was described as yelling about the resident taking her glasses and later bragging and laughing about what she had done. The administrator stated the roommate had been talking nonsensically and was sent for psychiatric evaluation after police and emergency medical services were notified. The resident was moved to a different room and continued to receive support from facility staff. The facility also failed to have a behavior-focused care plan in place for the roommate before the incident, despite a psychotropic evaluation documenting chronic behavior potentially causing harm to self or others. The roommate’s current care plan lacked documented behavior interventions until after the altercation. The report states that the resident-to-resident physical aggression occurred in the setting of these missing care plan interventions and that the facility had not documented psychosocial risk interventions for the roommate prior to the event.
