The facility failed to provide adequate supervision for one resident reviewed for elopement. The resident’s face sheet documented diagnoses of unspecified dementia, mild intellectual disabilities, and anxiety disorder. An elopement assessment completed on 5/7/26 documented that the resident was cognitively impaired, had poor decision-making skills, displayed behaviors indicating an attempt to leave, and that an elopement care plan was initiated. The administrator stated it was unclear who allowed him outside, the camera footage had a glitch, and there was no note in the medical record documenting the event or the resident’s assessment afterward.
