Summary

The facility failed to maintain residents’ right to dignified care for two residents. R10, who had diagnoses including unspecified dementia, psychotic disturbance, mood disturbance, anxiety, type II diabetes mellitus without complications, and lack of coordination, had an MDS documenting a BIMs score of 7/15 and need for staff assistance with all ADLs. On 04/12/2026, R10 was observed seated in a low wheelchair at a dining table that was level with her neck, making her food out of reach. R10 had to raise her arms to reach the plate, pull the plate partway off the table, lean forward to scoop food, and lean back to eat. She dropped food in her lap and stated she would like a lower table because she is short, cannot reach the food, and gets messy. A Dietary Assistant confirmed the table height made it difficult for R10 to reach her food and dine with dignity, and stated the table did not adjust because it was broken. The DON and Administrator later confirmed the low table was a dignity issue and that R10 needed a shorter table to dine comfortably. R13, who had hemiplegia and hemiparesis following cerebral infarction affecting the left non-dominant side, had an MDS documenting a BIMs score of 15/15 and total dependence on staff for toileting. On 04/13/2026, R13 was observed propelling herself out of the dining room and down the hallway in a wheelchair using her right foot and right hand, with a flaccid left side and one footrest holding her left foot. R13 stated she had asked a CNA for help to the bathroom, but the CNA said she did not have time because she was busy taking other residents to the dining room and starting feeding. R13 stated she could not sit in the dining room knowing she would urinate in her incontinence brief, could not unfasten it herself, and was afraid she would not make it to the toilet. The CNA confirmed she told R13 she could not take her to the bathroom and that she would have to wait, and the DON confirmed this was a dignity issue.
