Summary

The facility failed to ensure that a cognitively impaired resident with dementia and a documented history of elopement risk remained safe and supervised, resulting in the resident exiting through a facility door into the community and being missing for approximately 5 hours. The resident’s diagnoses included dementia, hypertensive heart disease, myocardial infarction, and chronic diastolic heart failure. The resident’s MDS showed a BIMS score of 10, indicating moderately impaired cognition, and records documented repeated exit-seeking behavior, including standing by the elevator and stating a desire to go home. The resident’s elopement risk assessment and care plan identified the resident as at risk to elope and indicated use of a wander guard. Facility documentation also reflected prior concerns about wandering, confusion, agitation, and attempts to leave the floor unsupervised. Despite this, the resident’s wander guard had been removed after a later assessment determined the resident was no longer at risk, even though other records continued to describe exit-seeking, impulsive behavior, and poor insight and judgment. Staff interviews stated that the resident had always been exit seeking, needed constant redirection, and had gotten off the unit multiple times before the elopement. On the day of the incident, staff reported that the resident was last seen on the unit and later found absent. A nurse initiated a code pink, conducted a head count and search, and staff eventually discovered that a first-floor door was slightly open. The administrator stated that camera review showed the resident exited through the first-floor door, and the exit alarm could only be heard locally near the door. The resident was later located by police wandering approximately 10 miles from the facility and was taken to the hospital for evaluation. Additional staff interviews noted that some wander guards were not functioning properly, keypad codes were written on top of the keypads, and residents could potentially enter the codes or access the elevator and exit areas.
