Summary

The facility failed to safely transfer a resident using a mechanical lift for 1 of 3 residents reviewed for falls. The resident had an admission history that included unspecified dementia with psychotic disturbance, severe dementia with agitation, generalized anxiety disorder, postural kyphosis, polyosteoarthritis, and muscle weakness. The resident’s MDS documented a BIMS score of 00, indicating the interview could not be completed, and noted short- and long-term memory problems.
