Summary

The facility failed to prevent the development of a facility-acquired stage 3 pressure ulcer on a resident admitted with multiple diagnoses including type 2 diabetes, multiple sclerosis, hypertension, GERD, low back pain, difficulty walking, unsteadiness on feet, and dementia. The resident was cognitively impaired and dependent on staff for toileting, hygiene, bathing, and transfers. Her care plan identified a potential for skin impairment and included interventions to keep skin clean and dry and to turn and reposition at least every two hours and as needed. Admission documentation and an early wound note indicated her skin was intact on admission and that there was no existing pressure ulcer. Family and staff interviews described that the resident later developed an open area on the sacrum that was first brought to nursing attention by a family member when the resident complained of pain and appeared uncomfortable. The family member stated she turned the resident and observed two sores on the sacral area, then notified an RN. The RN stated she assessed the sacral area and observed an open area with no dressing present, and the CNA was instructed to provide incontinence care and apply barrier cream. A CNA later stated she observed a skin impairment on the sacral region during incontinence care and reported it to the nurse, but the nurse did not assess the wound while she was providing care and there was no dressing covering the wound at that time. Wound care documentation identified the sacral wound as a stage 3 pressure ulcer in an area of previous scar tissue, described as a cluster and measuring 3.0 cm x 1.3 cm x 0.2 cm. The wound care nurse practitioner stated the resident was at increased risk for skin breakdown due to impaired mobility and that prolonged pressure to the sacral area and immobility contributed to the development of the stage 3 pressure ulcer. The nurse practitioner also stated earlier identification of the wound would have prevented its progression. The facility did not have documentation showing the resident was repositioned every two hours per the care plan, and the DON stated staff were expected to monitor skin during shower days and full body assessments and report changes promptly.
