Summary

The facility failed to properly assess and obtain treatment orders for a resident’s new skin condition after new areas were identified during care. The resident had a care plan for left and right lower extremity venous stasis ulcers, but no other wounds were documented. During a shower, new skin areas were noted, including slight redness in the front vaginal and perineum area and inside red, skin-peeling open areas on the buttock area. A subsequent skin check documented a small area to the right labia as a new skin impairment, but the form also indicated that treatment was not obtained and the care plan was not reviewed or updated. The resident’s record for the period reviewed did not document any new wound assessments, treatment orders, or care plan updates after the new skin conditions were discovered. Staff interviews showed the new skin findings were not escalated or addressed. The RN who completed the skin check stated she did not notify anyone after documenting the right labial tear and was not aware whether the finding was reported to the Administrator in Training or DON, or whether an order for treatment was obtained. The DON stated she was not made aware of the resident’s new skin concerns before the transfer to the hospital and was not aware of a labial tear. The resident was later transferred to the hospital, where the emergency room note documented scattered abrasions on the back, open areas from excoriation in the lower abdominal fold and up on the labia, and minor bleeding. The hospital RN stated the resident had a slit in the groin and another on the labia that were both untreated, with the groin wound described as deep and blood noted in the underwear.
