Gloves Not Changed Between Dirty and Clean Tasks During Incontinence Care

Alden Terrace Of Mchenry RehabMchenry, Illinois Survey Completed on 04-16-2026

Summary

The facility failed to ensure gloves were changed when moving from a dirty area to a clean area during incontinence care for two residents. During observation of care for one resident with hemiplegia and hemiparesis, metabolic encephalopathy, type 2 diabetes mellitus, dysphagia, legal blindness, and frequent bowel and bladder incontinence, two CNAs provided peri-care while wearing gloves. One CNA cleaned the resident’s groin, under the penis and scrotum, and wiped bowel movement from under the scrotum toward the anus without changing gloves, then placed a clean incontinence brief and assisted with repositioning and fastening the brief. The resident’s care plan showed dependence for toileting hygiene and personal hygiene and frequent incontinence of bowel and bladder. During observation of care for a second resident with cerebral palsy, dysphagia, obstructive and reflux uropathy, urinary retention, and dependence for activities of daily living, two CNAs transferred the resident to bed and provided incontinence care while wearing gloves. One CNA cleaned the groin, under the penis and scrotum, and then cleaned bowel movement from the buttocks and anus without changing gloves before handling the clean brief and fastening it in place. Staff interviews stated gloves are to be changed when going from dirty to clean and after peri-care. The resident’s care plan showed bowel incontinence and the MDS indicated dependence for activities of daily living.
