Unlabeled family food left in resident refrigerator with missed temperature checks
Summary
The facility failed to follow its policy for foods brought in by family or visitors for one resident who was reviewed for food safety. On 5/5/26, the resident was observed in her room with a counter-size mini refrigerator beside her bed. The refrigerator door had a handwritten sign dated 4/1 stating not to take anything out of it or throw anything away. With the resident’s permission, the surveyor inspected the refrigerator and found the small freezer section thawed and dripping water over the contents. Inside were a plastic container with an unidentified liquid, a wet baggie of meat identified by the resident as soup and lunch meat, a baggie of cinnamon bread, and a baggie of croissants, all wet. None of the foods had dates or labels. A baggie of grapes and a bottle of syrup on the inside door also had no dates or labels, and a packet of French dressing showed an expiration date of 10/9/25. The refrigerator temperature log for May 2026 stated that temperatures should be 36-46 degrees and that staff should circle the temperature and initial daily, but the log was blank for 5/2, 5/3, and 5/5. On 5/6/26 and 5/7/26, the temperature log and food items remained the same. The grapes had a white, creamy substance on the outside. The resident said her sister brings food occasionally, could not recall how often or how long the food had been in the refrigerator, and stated she was losing her eyesight and had trouble reading or seeing clearly. CNAs stated nursing staff did not monitor resident room refrigerators and that housekeeping was responsible. The Housekeeping Supervisor was unsure who monitored the refrigerators but said it should be done daily. The Administrator and ADON stated housekeepers should check and document refrigerator temperatures daily, and nursing staff should review food brought from the resident’s family and date it the same day. The facility’s policy stated perishable foods must be destroyed after 3 days, and the resident refrigerator policy stated temperatures must be checked at least once daily with a log maintained.
