Summary

The facility failed to ensure the planned menu was provided to residents. On 5/19/26, the lunch menu on the week-at-a-glance plan listed chicken breast supreme, carrots, green beans, and peach slump cake, but residents were served a chicken patty or beef patty, corn, green beans, and cake with whipped topping. A Culinary Assistant stated the chicken thighs had not been taken out of the freezer the night before, there was not enough time to thaw and cook them, and she substituted chicken patties and beef patties. She also stated the planned peach slump cake was altered by adding crushed canned peaches to the cake mix. On 5/20/26, the planned lunch menu listed meatballs with a sweet and savory sauce, rice, and green peas, but residents were served meatballs without sauce, white rice, and green beans mixed with green peas. A Culinary Assistant stated she used teriyaki sauce and orange marmalade to make a sauce because that was all she had available, and she added green beans because she would have run out of vegetables. The Culinary Manager stated the food truck had been arriving later than usual for the past few weeks, that the facility had to buy meatballs and peas from a local grocery store, and that not enough peas were purchased so green beans were added to feed the entire census. On 5/21/26, the planned lunch menu listed savory baked chicken, company potatoes, garden seasoned carrots, and chef's choice of dessert, but residents were served baked chicken thighs, rosemary potatoes, seasoned carrots, and cake because the company potatoes were not made due to not having enough fresh baking potatoes. During resident council, a resident stated the facility did not always follow the menu and residents did not know what would be served.
