Summary

The facility failed to ensure call lights were within reach and working for 4 of 8 residents reviewed for call lights. On 6/6/26 at 9:35 AM, R1’s call light was on the floor behind his recliner, and his wireless call light was tied to the bed post on the far side of the recliner, leaving him unable to reach either device. R2 was in her wheelchair with two call lights draped over her nightstand out of reach; she stated she did not have a way to call for help because she had been told the call lights were not working, and she said she never received a wireless call light. R4 was in bed with the head of bed raised, and both his regular call light and wireless call light were tied or draped on a chair and not reachable. R8’s call light was on the floor next to his bed, and his wireless call light was tied to his bedside table, also out of reach. During interview on 6/6/26 at 12:06 PM, two CNAs stated residents use call lights to call for help and they should always be within reach. Later that day, the Administrator stated there were issues with call lights not working or not being within reach and that all residents have both a traditional call light and a wireless call light. The facility’s call light response policy stated all residents will have a functioning call light, call lights must be checked at the beginning of each shift and after repositioning or transfer, and staff will ensure the call light is within reach during routine rounds.
