Summary

The facility failed to provide staff and resident access to an appropriate handwashing facility for a resident on contact isolation precautions for suspected Clostridioides difficile infection. The resident’s care plan documented an admission date of 5/15/2026 and a diagnosis of Enterocolitis due to C-Diff infection. On 6/1/2026, contact isolation signage was posted at the room entrance, a fecal odor was noted in the room, and the bathroom door was locked, preventing access to the sink for handwashing. The resident stated the bathroom had been locked by the facility and that staff had provided a bedside commode and alcohol-based hand sanitizer while on isolation precautions. During care on 6/2/2026, an LPN donned gown, gloves, and a mask before entering the room, administered Vancomycin to the resident, doffed PPE into the red disposal bin in the room, and then used alcohol-based hand sanitizer at the medication cart outside the room because there was no nearby area to wash hands with soap and water. The DON stated the resident was placed on isolation precautions on 5/30/2026 due to suspected C-Diff and that staff providing care did not have access to an appropriate handwashing facility following resident care. The facility also failed to implement Enhanced Barrier Precautions for a resident with an indwelling urinary catheter: the catheter drainage bag was observed lying on the floor under the wheelchair, no Enhanced Barrier Precautions sign was posted outside the room, and PPE was not readily available. The resident had a physician order dated 10/16/25 for Enhanced Barrier Precautions related to the indwelling urinary catheter, and the DON stated the precautions should have been implemented.
