Summary

The facility failed to maintain an effective pest control program to prevent and control insects within the building. The facility’s pest control policy stated that pest control was to be conducted on a regular and as-needed basis, staff were to promptly report pests, the facility was to be maintained to prevent harborage or feeding of insects and rodents, and a pest control log was to be maintained documenting the type of pest, location, and date observed. However, the maintenance director stated he was unaware of a pest control log, and the administrator stated he had considered using the contracted pest control company for additional spraying because ants were present in the facility. During observations and interviews, insects were found in multiple resident areas and common spaces. One resident’s room had food debris scattered throughout, with ants on the floor and flies in the room. Another resident was observed swatting flies in a shared room where flies were present. A cognitively intact resident reported ongoing crawling black bugs and flies throughout the facility, including in the resident’s room and hallways, and black flying insects were observed near a full trash receptacle in that room. Other residents reported ants and flying insects in rooms, hallways, and the dining area, including insects flying around faces during meals and ants crawling on the dining room floor. A CNA stated the ant and bug issue had been ongoing for several weeks and had been seen in several resident rooms and common areas.
