Summary

The facility failed to ensure consistent access to fluids to maintain adequate hydration for one resident who was cognitively intact but totally visually impaired, with glaucoma and blindness in the left eye. On 6/15/26, the resident was observed sitting in a wheelchair in front of the television and calling out for a drink of water. The water cup on the over-bed table was only half full, but the table had been pushed against the wall next to the bedside table and was completely out of the resident’s reach. Although the call light was in the resident’s lap, it was not used to summon assistance. The resident’s MDS documented little to no cognitive impairment, and the care plan identified the resident as visually impaired with an intervention to tell the resident where items were placed and to be consistent. When the DON was informed, the table was repositioned in front of the wheelchair and the resident was verbally oriented to the cup’s location using the clock-face method, after which the resident drank about 150 mL of water. The resident later stated, “I can't always find my water. Staff do get it for me when I ask.” On 6/17/26, the resident was again observed with the over-bed table pushed against the wall and out of reach until the DON repositioned it and informed the resident of the drinking cup’s location.
