Summary

The facility failed to provide 1:1 supervision during a meal for a resident with physician orders for 1:1 feeding assistance and swallowing precautions. The resident’s diagnoses included Huntington’s disease, dysphagia, dementia, lack of coordination, and gait/mobility abnormalities. Records showed the resident was at risk for aspiration and had orders for a mechanical soft diet with 1:1 feeding assistance, small bites, slow feeding, alternating liquids and solids, and ensuring the mouth was cleared before another bite was taken. Speech pathology notes documented that the resident required maximum assistance at meals because she could be impulsive, held food in her mouth at times, and needed staff to monitor her rate of eating and oral clearance. Staff interviews confirmed that the resident required 1:1 feeding assistance and cueing to slow down, take smaller bites, swallow, and take sips of liquid. The director of nursing stated the resident was expected to have 1:1 feeding assistance when food was presented. On the day of the event, a CNA escorted the resident back to the dining room after a neurology appointment and placed her tray in front of her while another CNA was feeding a different resident. The CNA stated she was not aware whether the resident required 1:1 feeding assistance. Facility camera footage showed the resident sitting alone at the table with food in front of her, moving food toward her face, becoming increasingly animated, and then becoming motionless. Staff did not assist her until approximately 13 minutes later, when an activity aide attempted to arouse her and called for help. Staff then moved her to the hallway, initiated CPR, suctioned her mouth, and rescue efforts continued until paramedics arrived. Interviews with staff and paramedics indicated there was uncertainty about whether the resident had been choking, and the resident’s tray afterward showed crumbs of biscuit and an empty baggie.
