Summary

He reported that he had submitted grievance forms about his state ID, community pass, and missing items, but said he had been waiting since February 2026 for answers and had not received clear explanations or resolution from social services, nursing staff, or the administrator. The resident’s concern form documented complaints about no follow-up on his green pass, needing help replacing a Chime card and ID, and missing items after a room change.
