Missed wound treatments for residents with pressure ulcers

Summary

The facility failed to ensure physician-ordered wound treatments were completed as ordered for three residents with pressure-related skin issues. R1 was admitted with diagnoses including infection and inflammatory reaction due to cardiac vascular devices, implants, and grafts, and end stage renal disease. His MDS documented that he was at risk for pressure ulcers and had two unhealed unstageable pressure ulcers. His care plan addressed altered skin integrity related to a deep tissue injury to the right lateral foot and right 5th toe, as well as a healing surgical wound to the right upper inner arm. The order summary and TARs showed ordered cleansing, betadine application, and leaving the wounds open to air, but multiple shifts in March, April, and May 2026 were left blank for the ordered treatments to the right 5th toe and right lateral foot. R19 was admitted with diagnoses including acute kidney failure, dementia, and cerebral infarction. His MDS documented a BIMS score of 00 and that he was at risk for pressure ulcers. His care plan included interventions for skin integrity risk, including incontinence care, barrier cream, turning and repositioning every two hours, pressure-reducing devices, and treatment orders. The order summary and TAR documented an order to cleanse the right hip wound, apply calcium alginate, and cover with a dry dressing daily, but the TAR was blank on several occasions in May 2026. R20 was admitted with diagnoses including Alzheimer's disease, chronic kidney disease, pressure-induced deep tissue damage of the left heel, and reduced mobility. His MDS documented severe cognitive impairment, dependence for rolling, risk for pressure ulcers, and at least one unhealed pressure ulcer. His care plan included treatment orders and turning and repositioning every two hours. The TAR documented ordered betadine application and foam dressing to the right heel for pressure-induced deep tissue damage, first with one dressing order and then with a revised dressing order, but multiple dates in May 2026 were left blank for the ordered treatments. Staff interviews confirmed they worked on some of the dates when treatments were not marked completed, but they could not explain why the treatments were not documented as completed and stated they might have forgotten to initial the TAR or may not have completed the treatment.
