Summary

The facility failed to ensure safe transfers and adequate supervision for two residents. One resident had been admitted for rehabilitation after all five toes on the right foot were amputated due to peripheral vascular disease, lower leg cellulitis, and diabetes. Hospital records showed the resident was non-weight bearing on the right foot, and the surgical wound was closed with sutures intact. On 6/8/26, the resident’s wife and daughter stated they saw the resident place the right foot on the floor and bear weight during a transfer into bed, and the daughter noticed bleeding on the gauze dressing afterward. The wound nurse later observed a small amount of bright red bleeding from the right foot incision, and the wife and nurse practitioner were notified. The CNA involved stated the resident kept putting the right foot down during the transfer and staff continued the transfer anyway. The ADON stated staff should have stopped the transfer and notified the nurse if the resident was bearing weight on that foot. The facility also failed to follow the resident’s transfer and toileting assistance needs for another resident who was at risk for falls due to weakness, dementia, glaucoma, prior falls, and impulsive behaviors. The care plan and restorative assessment showed the resident required one staff member and a gait belt for all transfers. During observation, a CNA placed a gait belt on the resident but transferred her by lifting under the armpits instead of using the gait belt, then transferred her onto the toilet by holding the sides of her incontinence brief. The CNA then left the resident alone and unsupervised on the toilet. The restorative director stated staff are to hold the gait belt during transfers, not the resident’s arms or waist, and stated the resident should not be left alone while toileting because of her impulsive behaviors and fall risk.
