Summary

The facility failed to provide ordered pain medications for 3 of 4 residents reviewed for pain management. R101, who had diagnoses including acute hematogenous osteomyelitis of the left ankle and foot, cellulitis of the left lower limb, and a history of bronchus and lung cancer, had an order for Hydrocodone-Acetaminophen 10-325 mg every 6 hours as needed for pain starting 6/4/2026, but the MAR showed no pain medication administered from 6/4/2026 through 6/9/2026 at 9:30 AM. R101’s wife stated he had not had his pain medication for 2 days and needed it, and R101 agreed. R101 later stated that during the days the medication was unavailable, his back pain reached an 8 on a 1-10 scale and that the medication controlled his pain. R84, who was cognitively intact and had a care plan addressing pain, had an order for Tramadol 50 mg every 6 hours for an unspecified fracture of the wrist and hand. The medication monitoring/control record documented multiple missed administrations, including several dates in April and May and a stretch from 5/20/2026 through 5/30/2026. R84 stated he had a lot of pain, that when he did not receive Tramadol his pain was excruciating, and that he had been out of the medication for about a week. He stated the medication only took the edge off, that the last pill was given on a Tuesday and the medication did not come in until Sunday, and that it was hard to get out of bed. R5 had orders for Cyclobenzaprine 5 mg every 8 hours as needed for muscle relaxer, Metoprolol Succinate ER 25 mg daily, and Levothyroxine 88 mcg in the morning. During a medication pass, the Levothyroxine and Metoprolol scheduled for 8 AM were unavailable and not given, and R5 requested Cyclobenzaprine but it was not administered because it was not in the cart and needed to be reordered. R5 stated that when she does not get her medication as ordered, her anxiety increases and behaviors become hard to control, and that she has severe phantom pain and excruciating cramps and spasms in her left leg and toes. Staff stated the medication was not in the cart, had run out, or was not available, and the administrator stated the pharmacy usually had a 24-hour turnaround or an emergency run was possible, and that residents should have medications as ordered.
