Summary

The facility failed to maintain the resident call light system in working order for 4 of 5 residents reviewed for call lights in a sample of 13. On 6/15/26, R1 and R2 were observed seated in bed in their shared room, and both stated their call lights had not worked for weeks or a long time. When their call buttons were pressed, neither call light lit up or beeped. The shared bathroom call light was also tested and was not working. Later, when R1’s call light button was pressed again, the nurse station monitor did not display the room number and did not beep, and the LPN stated the light probably did not work and that a new call light system was needed because this happened a lot. R13 also reported that her call light had not worked when she activated it the prior night and no staff responded. She told the LPN that morning that the call light was not functioning, but the LPN had not yet checked it or reported it to Maintenance. When R13 was asked to activate her call light, the nurse station monitor did not light up or show that R13 or her roommate’s call light had been activated. The facility’s policy stated that call lights should be available at each resident’s bedside, toilet, and bathing facility and should directly relay to staff or a centralized location for response.
