Improper glove use during meal service

La Bella Of WoodstockWoodstock, Illinois Survey Completed on 06-10-2026

Summary

The facility failed to ensure that dietary staff changed gloves during food service after contacting potentially dirty objects and areas. During observation on 6/9/26 at 11:40 AM, the Head Cook was checking food temperatures at the steam table while wearing gloves, then touched his glasses, rubbed the side of his nose, opened the service window door, rubbed his eye, stacked plates and bowls, rested his gloved hand on a plate while speaking with the surveyor, handled a notebook used for collecting food temperatures, and touched his glasses again, all without removing or changing the gloves. The Head Cook then began serving the noon meal with the same gloves still on. The Dietary Manager asked for a chopped chicken piece, removed chicken from the steam table pan, and placed it on the counter; the Head Cook cut the chicken and scooped it with his gloved hand onto a plate. The Dietary Manager stated that gloves should be changed between everything done during food service and that the servers do not wear gloves, but dietary staff do when serving from the steam table and should change them between tasks. A resident also stated that the Dietary Manager was always touching his face with gloves on and then serving food. The facility policy on glove use for food handling states that gloves should be discarded when damaged, torn, soiled, or when interruptions occur or when handling something beyond the scope of food prep.

The maintenance supervisor said he was responsible for cleaning and maintaining the machine, changed the filter monthly, visually inspected it weekly, and deep cleaned it in July, but may have missed part of the guard.
