Summary

The facility failed to follow physician orders for dating and labeling oxygen tubing and humidifier bottles for three residents who were receiving oxygen therapy. R66 had diagnoses including COPD and centrilobular emphysema, and the physician ordered weekly changes of oxygen tubing and humidifier bottle with initials and date once a day on Saturday. Although the MAR documented weekly changes, the tubing observed on 4/15/26 had a label with a date of 3/14 and no time, and the humidifier bottle was not labeled with a date or time. The regional nurse confirmed the tubing date was 3/14 and that the tubing and bottle should be changed weekly and labeled as ordered. An LPN stated nursing was supposed to date and label the oxygen tubing when changed, and the regional clinical staff stated the equipment company services or switches out equipment as needed, but the tubing and bottle still should be dated and labeled when changed. R64, who had diagnoses including acute and chronic respiratory failure with hypoxia, secondary pulmonary arterial hypertension, and COPD, also had an order to change oxygen tubing and humidifier bottle weekly and initial and date once a day on Saturday. The MAR documented Saturday changes, but on multiple observations the tubing and humidifier bottle had no date or time showing when they were last changed. R64 stated the tubing was not getting changed as regularly as it used to and that one nurse would do it if working, otherwise it never got changed. R80, who had chronic respiratory failure and anxiety, had the same weekly oxygen tubing and humidifier bottle order with Saturday changes documented on the MAR, but the tubing and humidifier bottle were not labeled with the date and time of the last change when observed. The administrator stated the oxygen tubing should be labeled with the date and time of exchange as the order states.
