Summary

The facility failed to refer a resident for PASARR reevaluation after a diagnosis of Schizoaffective Disorder, Depressive Type was added to the resident’s record. R28’s face sheet showed an admission date of 10/31/17 and listed Schizoaffective Disorder, Depressive Type with an effective date of 9/7/24. The resident’s MDS documented moderate cognitive deficits, and the care plan identified antidepressant medication for appetite and Schizoaffective Disorder, Depressive Type. However, the Illinois Department of Healthcare and Family Services Interagency Certification of Screening Results dated 10/31/17 indicated that nursing facility services were appropriate and that there was no reasonable basis for suspecting MI or DD, and there was no documentation that the resident was referred for reevaluation after the new diagnosis was added. During interview, the Behavioral Health Case Manager stated the resident had a long history of mental illness from prior community mental health services and had initially been admitted for skilled nursing care before later moving to the behavioral health unit. The Administrator stated there may be other residents in the facility who need to be rescreened for PASARR and said she would audit and have those residents rescreened. The facility policy stated that any resident who exhibits a newly evident or possible serious mental disorder, intellectual disability, or related condition will be referred promptly to the state mental health or intellectual authority for a level 2 resident review.
