Summary

The facility failed to provide a clean, homelike environment for 2 of 2 residents reviewed for cleanliness. One resident with diagnoses of hemiplegia and hemiparesis following cerebral infarction affecting the right dominant side, and with severe cognitive impairment documented on the MDS, was observed self-propelling in a wheelchair that was in disrepair. The wheelchair had a bandage wrapped around both sides of the frame near the seat, and the frame was observed with red sauce, old cheerios, hair, and dirt. The resident’s non-slip grip under the cushion also had food stains on the outside of the cushion. The resident’s care plan noted unclear speech and difficulty finding words, with interventions to observe closely and anticipate needs as needed. A second resident’s bed frame was observed to be visibly soiled with dried food and spills, and the same condition was still present on a later observation. The Administrator stated that the resident likes to eat and drink in bed and spills a lot of food and is a messy eater, and said someone would be brought in to clean the bed frame. A CNA stated that night shift nursing staff are supposed to clean wheelchairs but that it was not being done, and also stated there was no documentation for the wheelchair being cleaned and did not know why there was a bandage on it.
