Failure to Administer Ordered Pneumococcal Vaccines

Summary

The facility failed to provide pneumococcal immunizations for 4 of 9 residents reviewed for immunizations in a sample of 51. R18, R47, R62, and R64 were all over the age threshold referenced in the report and had diagnoses including dementia, depression, anxiety, hypertension, COPD, heart failure, and other chronic conditions. I-Care records showed no pneumococcal vaccines for R18, R62, and R64, while R47 had prior pneumococcal polysaccharide and Prevnar 13 vaccinations documented. Each of these residents had vaccine consent forms dated only by year 2026 and progress notes stating consent was obtained and orders were entered for Prevnar 20, but the vaccines were not administered at the time of the survey review. On observation of the medication room, surveyors found Prevnar 20 vaccines stored in the medication refrigerator for all four residents. The order dates on the vaccines were in March 2026, yet the vaccines remained in the refrigerator and had not been given. An LPN stated the vaccines had not been administered because the consents were not scanned into the chart. Another staff member stated she believed the residents now had consents. The DON stated that when the medical records employee quit, there was a pile of papers that needed to be scanned into the chart and that this had not been done. The DON further stated that nurses would not give Prevnar 20 until the consents were scanned into the chart. She also stated she had recently started at the facility, had just gained access to I-Care, had not yet reviewed who was up to date or needed vaccines, and was not aware that vaccines were in the medication refrigerator awaiting administration. The Administrator stated it was her expectation that vaccines be given when ordered and that staff follow the vaccination policy, but she was not aware the consents had been obtained but not scanned. The facility policy stated residents should be protected from incident of pneumonia by obtaining pneumococcal vaccines per CDC guidelines.
