The facility failed to complete and transmit quarterly MDS assessments within the required timeframe for 4 residents in the sample. R13, admitted with unspecified dementia, type 2 diabetes mellitus, Alzheimer's disease, chronic kidney disease, essential hypertension, hypothyroidism, delusional disorder, and B12 deficiency anemia, also had a quarterly MDS submitted on 04/14/2026 with a target/due date of 03/05/2026 and the same late-completion warning. R51, admitted with spinal stenosis, low back pain, polyosteoarthritis, cardiomegaly, generalized anxiety disorder, and essential hypertension, had a quarterly MDS submitted on 04/14/2026 with a target/due date of 03/03/2026 and a warning that it was completed late by more than 14 days after the assessment reference date. R56, admitted with unspecified dementia, generalized anxiety disorder, hypothyroidism, depression, and urinary tract infection, had a quarterly MDS submitted on 04/14/2026 with a target/due date of 03/06/2026 and the same late-completion warning. The Regional Nurse stated the facility did not have an MDS nurse and that a regional nurse was trying to complete all MDSs. The Administrator stated a former employee quit without notice on 03/24/2026, she was not aware the employee was behind on MDS assessments, the company did not have a policy on when MDSs are to be completed, and they follow the RAI Manual.
