Summary

The facility failed to perform weekly assessments for a pressure injury and failed to have an ordered pressure injury treatment in place for a resident admitted with multiple diagnoses including Parkinson's disease, hypertension, and altered mental status. An open area to the sacrum was first documented as 1 cm by 1 cm and treated with cleansing, xeroform, and padding, but the record showed no further wound assessment after that initial note. The wound later progressed, and the wound doctor documented an unstageable pressure wound to the sacrum measuring 2.2 cm by 3.2 cm with light drainage. The DON stated the nurses should document measurements when an open area is identified, notify the wound nurse and physician, and complete weekly assessments, but she did not find weekly wound assessments following the initial finding. The facility also failed to have preventive interventions in place for a resident with bilateral heel pressure injuries and a large sacral wound. The resident was observed lying in bed with both heels resting on the mattress and the heel boots sitting on the couch, and the ADON stated the boots should have been in place to prevent further injury. The wound care provider documented end stage skin failure to both heels and recommended off-loading and daily skin prep. The resident also had a large full-thickness sacral wound with undermining, and the ordered treatment included sodium hypochlorite solution-moistened gauze packed into the wound with a dry dressing as a secondary dressing. A third resident was observed asleep in a chair wearing grip socks and athletic shoes while a cast shoe was on the bed, and staff stated the resident had a deep tissue injury to the right heel. When the shoe was removed, black necrotic tissue was present on the heel, and staff had difficulty reapplying the shoes over the heels. The wound care provider documented an unstageable full-thickness wound of the right heel present on admission and recommended skin prep and off-loading. The care plan noted weekly treatment documentation for the heel wound, but there were no preventive interventions documented for off-loading the heel.
