Summary

The facility failed to ensure residents were provided information on Advance Directives and failed to ensure that a POLST and emergency code status were documented in the medical record for three of four residents reviewed for Advance Directives. The facility policy stated that at admission the social service director would provide educational information regarding state and federal laws on Advance Directives, and that Advance Directives would be reviewed by the interdisciplinary team and addressed in the plan of care, physician progress notes, physician's orders, and social service progress notes. The policy also required a written physician's order for CPR status and that related physician progress notes and advance directive documents be maintained in the medical record. R46's physician order sheet documented admission to the facility but did not include a physician's order for code status, and the electronic medical record did not document an emergency code status or any advance directives. The DON verified that R46's record did not contain advance directives or code status, and the Social Services Director stated she discusses advance directives and code status on admission but did not believe the resident had advance directives in the record. R77 and R78 also had physician order sheets documenting admission without a physician's order for code status, and their electronic medical records did not document an emergency code status or any advance directives. The Administrator stated R77 and R78 did not come to the facility with advance directives and were assumed to be full code, and confirmed the facility did not initiate an advance directive for either resident.
