Dirty and Sticky Resident Room Floors

Washington Senior LivingWashington, Illinois Survey Completed on 03-11-2026

Summary

The facility failed to ensure resident room floors were clean and free of debris for four residents reviewed for a clean and homelike environment. The Housekeeping Supervisor and Housekeeping job descriptions required the environment to be safe, clean, comfortable, and home-like, and the housekeeping staff to clean floors by sweeping, damp/wet mopping, and disinfecting. The housekeeping daily schedule showed reduced staffing on several days, and resident council meeting minutes documented concerns about short staffing, rooms not being cleaned in a timely fashion, and sticky floors in the assisted dining room. During observations, one resident’s room floor was covered in dried debris, red-colored liquid stains, and was sticky, and the same condition remained the next day. Another resident’s room had dried stains throughout the floor, was sticky, and had debris and thick debris around the wall base, with the same conditions observed the following day. A third resident’s room was dirty, sticky, and covered in debris with thick black debris in the corners, and these conditions remained the next day. A fourth resident’s room floor was covered in stains, sticky, and covered in pieces of food and dried debris, and it remained the same the next day. The Maintenance Director, who had been assigned to oversee housekeeping services, verified the rooms were dirty and stated the rooms should be swept and mopped at least daily and had not been. The Administrator stated there had been no Housekeeping Supervisor for the past 6 months and that staffing shortages may have contributed to rooms not being cleaned daily.

On the secured unit, all observed residents were served meals on plastic trays holding their plates, utensils, and drinks instead of having items placed directly on the table, and staff and the DON confirmed this was the routine practice.
