The facility failed to ensure that a working call system was available in residents' bathrooms and bathing areas, with call light communication systems not functioning in four of 25 resident rooms identified during observation. During an observation with the Maintenance Director, room 114A did not have a functioning call light because the light above the resident's door did not illuminate when the button was pressed, and room 109B also had a nonfunctioning call light with no alternative method for the resident to request assistance observed. In room 112B, the call light cord was broken and missing from the wall-mounted panel, and although a bell was present on the bedside table, it was not within the resident's reach. A replacement cord was brought in and installed during the observation, but the call light system remained nonfunctional. In room 201A, the call light did not illuminate outside the room when activated. Later observation showed the wall-mounted call light device in room 109B had been removed, a bell was present but still not within the resident's reach, and the Maintenance Director stated a vendor was scheduled to repair the system that day; at exit, the call light system had not been repaired.
