Failure to Provide Ordered Adaptive Drinking Equipment

Boynton Beach Rehabilitation CenterBoynton Beach, Florida Survey Completed on 03-26-2026

Summary

The facility failed to provide assistive eating equipment for 1 of 8 sampled residents, specifically failing to provide a 2-handled cup to Resident #108. The resident was admitted with diagnoses including Rheumatoid Arthritis, and a physician’s order dated 10/08/24 directed that the resident receive a 2-handled cup and scoop plate at all meals. The facility policy stated that resident-specific equipment is issued by the treating therapist, who provides instruction in its use and documents the resident’s response in the medical record. During breakfast observation on 03/24/26, Resident #108 had a 2-handled cup filled with orange juice and a regular coffee mug filled with tea. The resident stated it was difficult to drink tea from the coffee mug because it had only one handle and no lid with a spout, and said Rheumatoid Arthritis made it hard to hold things and that poor vision made the 2-handled cup with a lid easier to use for fluids. On 03/25/26, the resident was observed with tea placed in a 2-handled cup, but the orange juice was in a standard juice cup without handles, and the resident stated it would be difficult to drink from that cup. Staff B, an LPN unit manager, acknowledged that the orange juice should have been sent in a 2-handled cup, and the Culinary Manager stated she did not know the resident drank tea and that the orange juice should have been sent from the kitchen in a 2-handled cup.
