Summary

The facility failed to ensure resident rights for 1 of 2 sampled residents, Resident #96, by not providing showers in a dignified manner and by not maintaining reliable access to the call bell. Resident #96 was admitted with diagnoses including hemiplegia following cerebral infarction affecting the left non-dominant side and a right below-knee amputation. The current MDS documented a BIMS score of 12, indicating moderate cognitive impairment, and the care plan stated he required substantial/maximal assistance of one for bathing. Resident #96 stated that during showers, staff placed his whole wheelchair in the shower instead of using a shower chair, and that the wheelchair became soaking wet. Staff C, a CNA, confirmed she gave him showers while he remained in his wheelchair with the sling under him for the mechanical lift, removing the back and seat cushions so they would not get wet. She stated the shower chairs were not safe for him because he moved, slid down, leaned to the side, and lacked enough support. Resident #96 stated he did not think he had ever been showered in anything but his wheelchair and said it was not right because it stayed wet. Resident #96 also reported that his call bell did not always work because staff took it out of the wall. He stated he used it when he was in pain, cold and wet, or constipated, and that when it did not work he screamed for help. His significant other stated she had seen the call bell pulled out of the wall and that staff said someone must have pulled it out. Surveyor testing found the call bell was not fully plugged into the wall and did not activate until it was pushed in. Staff C and Staff E confirmed the call bell had been partially pulled out and stated it sometimes got coiled on the siderail and pulled out, while the significant other stated she sometimes found it that way in the morning.
