Summary

The facility failed to ensure residents received necessary wound care and skin monitoring. For one resident with Parkinson disease, dementia, immunodeficiency, muscle atrophy, and a history of cerebral infarction who required total assistance and was incontinent, a stage 4 sacral pressure injury was observed with a dressing dated four days earlier, and the resident was not wearing ordered heel offloading boots during observations. The physician had ordered daily sacral wound care and had recommended offloading the wound, turning and repositioning, offloading heels with pillows or heel boots, dietary evaluation, and consideration of a skin substitute, but the resident’s feet were resting directly on the mattress and staff confirmed the heels were not offloaded. For another resident with metabolic encephalopathy, muscle weakness, dementia, dysphagia, and severe peripheral vascular disease, the initial nursing assessment documented a right lower leg skin tear, but weekly skin checks later only noted an open area on the right lower leg without further description or treatment orders.
