Failure to Report Resident Elopement
Summary
The facility failed to report an unauthorized exit from a secure unit in accordance with State law and established reporting procedures for one resident. The resident had been admitted with diagnoses including unspecified dementia with agitation, lack of coordination, and muscle wasting and atrophy, and the admission MDS documented a BIMS score of 01, indicating severe cognitive impairment. The resident had been placed on the secure unit because of a history of exit seeking and attempting to leave another facility. According to the resident representative, the facility called to say the resident had escaped from the secure unit, and police later told the representative they found the resident a considerable distance from the facility. Staff stated the resident was able to leave the building during a fire drill because the fire alarm disabled the locked doors. Staff also stated the resident was missing for an estimated 15 minutes, the code was called, the police were notified, and the police located the resident and returned him to the facility. The nurse progress note documented the resident leaving the facility for a walk and later returning at baseline, with vital signs within normal limits and no signs or symptoms of injury. The facility’s reportable adverse and grievance log contained no elopement entry for January 2026. During interview, the NHA and DON stated they would report an unauthorized exit only if it rose to that level and said they were not sure whether an elopement would be considered an adverse event until after an investigation. They stated they did not report the incident because the resident was safe, could demonstrate safety awareness with traffic, could cross the road, and was dressed appropriately for the time of day. The facility policy stated that the administrator, DON, or designee are responsible for investigation and reporting of suspected or alleged abuse, neglect, exploitation, and misappropriation, and that an immediate report would be filed with AHCA for alleged violations involving abuse, neglect, exploitation, or mistreatment, including injuries or unknown source suspicious of abuse or neglect.
