Hot Beverage Served Without Temperature Check, Resulting in Burns

Summary

The facility failed to ensure procedures and safety measures were in place for the temperature and serving of hot beverages for a resident with left-sided weakness and significant assistance needs. The resident was cognitively intact, had upper and lower extremity impairment, and required varying levels of assistance with activities of daily living. During the early morning hours, a CNA prepared instant coffee by heating water in a foam cup in the microwave for about 2 minutes, mixed the coffee, placed it on the resident’s bedside table over his abdomen, and left the room after the resident requested privacy. Shortly after the CNA exited, the resident activated the call light and was found to have spilled the hot coffee on himself. The incident resulted in burns to the abdomen, left hip, and right lower back, with documentation describing erythema, blistering, and later wound assessments identifying full thickness wounds. The resident reported that the coffee was too hot and that he had previously experienced coffee being heated too much and burning his tongue. The resident’s records also showed he had diagnoses including cerebral infarction, intracerebral hemorrhage, cortical hemiplegia and hemiparesis, difficulty walking, muscle weakness, and dysphasia. The report also documented that the facility did not have a system in place at the time to check hot liquid temperatures before serving, and staff stated that thermometers were not available in the pantries until after the incident. Observation later showed coffee temperatures in the dining areas and on the floor exceeded the facility’s stated limit of 120 degrees Fahrenheit, and staff served coffee to the resident without checking the temperature before delivery. Interviews reflected that staff believed the incident could have been prevented and that the resident’s safety was compromised when the hot beverage was prepared and served without adequate temperature verification and supervision.
