Summary

The facility failed to ensure accurate administration of a narcotic medication for one resident when an LPN spilled approximately one-third of a crushed Percocet 5/325 mg tablet onto the medication cart while preparing it with applesauce for administration. The LPN then administered the partial dose to the resident instead of discarding it and obtaining a new dose. The resident had an order for Percocet 5/325 mg twice daily for pain related to a sacral wound and was observed moaning with facial grimacing and stating that they were in pain at the time of the event. The DON stated that the LPN should have disposed of the medication and obtained a new dose so the resident would receive the complete prescribed amount. The facility also failed to ensure reconciliation of narcotic medications for a second medication cart. During a review of the narcotic medication cards and logbook, four discrepancies were identified involving morphine sulfate ER, lacosamide, hydrocodone/acetaminophen, and oxycodone/acetaminophen for four residents. In each instance, the medication card count and the logbook count did not match, and Staff B stated the doses had been given but not signed out in the logbook. Review of the EMR showed the doses had been documented as administered earlier that day. Staff B stated she had worked a double shift and forgot to document the narcotic administrations in the logbook, and the DON confirmed the facility expected nurses to document medication administration in both the logbook and EMR at the time the medication is given.
